Pursuant to A.R.S. §38-431.02(K), individual Supervisors and the County Administrator may present brief summaries of current events but no discussion may occur and no action may be taken regarding anything that is presented.
 
PRESENTATIONS
 
1.   Board of Supervisors - Presentation to the Bagdad/Yarnell Justice of the Peace Anna Mary Glaab, in recognition and appreciation for her 35 years of service to Yavapai County.
 
2.   Board of Supervisors - Presentation to Mayer Constable Hank Hellman, in recognition and appreciation for his 28 years of service to Yavapai County.
 
3.   Board of Supervisors - Presentation of checks by the Board of Supervisors to the FY2018/19 Title III Forest Fee Program awardees.
 
4.   Community Health Services - Presentation by Carol Lewis, Section Manager, Community Health Education and Leslie Horton, Director, Community Health Services.
 
5.   Human Resources - Presentation of Service Awards to Elected Officials and Department Directors.
 
CONSENT AGENDA All items on the Consent Agenda were unanimously approved with the exception of item 21. 
 
1.   Board of Supervisors - Approve minutes of special meeting of November 15, 2018, and minutes of meeting of November 20, 2018.
 
2.   Board of Supervisors - Approve appointment of Lloyd (Al) West as a member of the Board of Adjustments and Appeals, representing District 4. Term set to expire June 30, 2021.
 
3.   Board of Supervisors - Approve appointment of April Rhodes as District 1 representative, term set to expire June 30, 2020, and appoint Tim Barnett as member at-large on the Board of Health, term set to expire June 30, 2021.
 
4.   Board of Supervisors - Approve State required revisions to the Yavapai County Workforce Development Plan 2016-2020 as mandated by the Workforce Arizona Council.
 
5.   Board of Supervisors - The Board of Supervisors will resolve into the Coyote Springs Road Improvement District and following consideration of the following item(s), will reconvene into the Board of Supervisors - Approve contract for billing services with Improvement District Services from January 1, 2019 through December 31, 2019. 6.   Board of Supervisors - The Board of Supervisors will resolve into the Poquito Valley Road Improvement District and following consideration of the following item(s), will reconvene into the Board of Supervisors - Approve contract for billing services with Improvement District Services from January 1, 2019 through December 31, 2019. 7.   Board of Supervisors - The Board of Supervisors will resolve into the Seligman Sanitary District and following consideration of this item(s) will reconvene into the Board of Supervisors - Approve contract for billing services with Improvement District Services from January 1, 2019 through December 31, 2019. 8.   Board of Supervisors - Approve vouchers for November 13, 2018 through November 23, 2018.
 
9.   Development Services - Approve Assistant Director, Jeremy Dye, to attend the National Planning Conference in San Francisco, California, held on April 13-16, 2019. (All Districts)
 
10.   Development Services - Approve renewal of a Use Permit for an additional five (5) years to allow for outside storage of heavy construction equipment and a waiver of the setbacks, on an approximate 2.4-acre parcel in an R1-2A (Residential; Single Family; 2-acre minimum) zoning district with the previously approved stipulations; APN: 402-15-075Q; H18047; Tom & Lisa Lowery. Located on the Southside of Old Cherry Road approximately 2,000 feet North of the intersection of S.R. 169 and Old Cherry Road in the vicinity of the Town of Dewey-Humboldt S33 T14N R2E G&SRB&M. (District 2 - Supervisor Thurman)
 
11.   Facilities - Authorize a fund transfer from Facilities to Public Works/Roads in the total amount of $10,773.00 ($10,260.00 with 5 percent contingencies of $513.00) for the chip seal of the dirt access road to Ash Fork Cemetery. (District 4 - Supervisor Brown)
 
12.   Fleet Management - Approve the purchase of twenty (20) replacement vehicles and equipment upfitting for thirty-nine (39) vehicles through the Arizona Counter Drug Procurement 1122 Program, competitive bidding, and in-house procurement contracts. (All Districts)
 
13.   Library District - The Board of Supervisors will resolve into the Board of Directors of the Yavapai County Free Library District, and following consideration of this item, will reconvene as the Board of Supervisors - Approve an Intergovernmental Agreement (IGA) with the Yavapai County Superior Court for the provision of library support services and participation in the Yavapai Library Network, effective July 1, 2018, for up to a maximum of five (5) one-year terms. YC Contract No. 2018-494 (District 1 - Director Simmons)
 
14.   Library District - The Board of Supervisors will resolve into the Board of Directors of the Yavapai County Free Library District and following consideration of this item will reconvene as the Board of Supervisors - Approve agreements between the Library District and the City of Cottonwood, YC Contract No. 2018-493; Orme School of Arizona, YC Contract No. 2018-491; Prescott College, YC Contract No. 2018-492; Sedona Public Library, Inc., YC Contract No. 2018-490; Town of Camp Verde, YC No. 2018-489 and Town of Prescott Valley, YC Contract No. 2018-495, effective July 1, 2018, to provide the delivery of library materials by the Library District in an amount to be determined annually via a formula defined in the agreement (District 1 - Director Simmons; District 2 - Director Thurman; District 3 - Director Garrison; District 5 - Director Smith)
 
15.   Public Works - Approve Cultural Resource Manager, Kathryn Turney, to attend the annual conference for the Society of American Archaeology in Albuquerque, New Mexico from April 9-14, 2019, and approve one County vehicle to be used for out-of-state travel.
 
16.   Public Works - Approve acceptance of a Quit Claim Deed for purchased right-of-way on West Hoffman Road in the Hassayampa Mountain Club Subdivision. (District 1 - Supervisor Simmons)
 
17.   Public Works - Approve acceptance of donated Water Utility Easement and Sight Distance Easement in the Prescott East Unit 3 Subdivision on APN 103-27-031A. (District 5 - Supervisor Smith)
 
18.   Public Works - Approve Change Order #1 (Final) in the decreased amount of ($62,631.96) with Cactus Asphalt, a division of Cactus Transport, Inc.for Dry Creek Road-Boynton Pass Road-Boynton Canyon Road Improvements in Yavapai County, AZ - Project #1822209. 19.   Public Works - Approve Change Order #1 (Final) in the decreased amount of ($49,877.74) with Asphalt Paving & Supply, Inc. for Bullock Road Reclamation in Yavapai County, AZ - Project #1722043. YC Contract No. 2018-150A1 (District 4 - Supervisor Brown)
 
20.   Public Works - Approve Change Order #2 (Final) to Authorization of Services #1620886 with Woodson Engineering & Surveying, Inc. in the decreased amount of ($19,995.16) for design services on Dry Creek Road/Boynton Pass Pavement Preservation. 24.   Sheriff - Approve one (1) employee to attend Street Crimes training on December 18-20, 2018, in Mesa, Arizona.
 
25.   Sheriff - Approve two (2) employees to attend the training for Force Science certification on January 14-18, 2019, in Scottsdale, Arizona.
 
26.   Superior Court - Approve re-appointment of Justices of the Peace Pro Tempore for the period January 1, 2019 through December 31, 2019.
 
27.   Superior Court - Approve the appointment of the following as Juvenile Hearing Officers pursuant to ARS §8-323: Shelly J. Bacon, Dennis Dowling, and Brandi Suitor.
 
28.   Superior Court - Approve Chief Justice Scott Bales' appointment of Debra R. Phelan to the position of Judge Pro Tempore, Division A, effective January 7, 2019, pursuant to A.R.S. §12-141.
 
29.   Superior Court - Approve two (2) court employees to take a County vehicle out of state to attend the e-courts annual conference on December 9-12, 2018, in Las Vegas, Nevada.
 
ACTION
 
1.   Board of Supervisors - Appoint Chairman and Vice Chairman of the Board of Supervisors for calendar year 2019, to be effective January 1, 2019. Approved
 
2.   Human Resources - Approve an agreement with Dr. Maria Church, Government Leadership Solutions, for mandatory leadership development training for Appointed Department Directors and authorize the Human Resources Director to sign all required documents. Approved
 
3.   Human Resources - Discussion and possible action on position reclassifications and new job descriptions as requested by County departments, for an effective date of January 13, 2019. Approved by 4 to 1 vote. Supervisor Brown opposed. Approved

1.   Development Services - Recommend approval to the Arizona Department of Liquor Licenses and Control of a Series 6 Liquor License application (36889) without protest submitted by Philip Sacramone for Arrowhead Bar & Grill located at 25575 N. 2.   Development Services - Recommend approval to the Arizona Department of Liquor Licenses and Control of a Series 10 Liquor License application (33544) without protest submitted by Anna Karobowniczek for Paulden Park Place located at 23310 N. 4.   Development Services - Approval of an Extinguishment of a PAD (Planned Area Development) to the underlying RCU-2A (Residential; Rural; 2 acre minimum lot size) zoning district consisting of approximately 102 acre portion of the 329 acre project known as the Antelope Creek Subdivision, Development Services Staff; Excelsior Trust; APN: 201-17-011J; HA# H18044. 8.   Library District - The Board of Supervisors will resolve into the Board of Directors of the Yavapai County Free Library District and following consideration of this item will reconvene as the Board of Supervisors - Courtesy hearing for the adoption and/or modification of the following policies: Animals in the Library, Bulletin Board Usage, Code of Conduct, Collection Development, Confidentiality of Patron Records, Gifts and Donations, Internet/Computer/Wireless Usage, and Unattended Children and Vulnerable Adults. No Action
